What the Job Involves
The primary role is to work as a care management social worker assessing the needs of vulnerable adults and their carers, promoting independence, self-directed support, and safeguarding. You will carry out care management assessments, assist in service planning, and support multi-agency teams for seamless health and social service delivery. Responsibilities include ensuring adherence to risk management and safeguarding procedures, engaging in support planning, managing casework, and utilizing community resources for enhanced personal wellbeing.
