Who You Are

The Progression and Next Steps Social Worker supports adults with learning disabilities, autism, and/or complex needs to achieve positive outcomes. The role is focused on strengths-based practice to promote independence and facilitate transitions towards greater autonomy, wellbeing, and inclusion. The social worker aligns reviews and planning with the individual's aspirations and assessed needs, supporting the service's aim of reducing dependency and improving quality of care.

What the job involves

  • Conducting high-quality assessments, reviews, and developing person-centred support plans.
  • Proactively working with individuals, families, and providers to foster skill development and independence.
  • Collaborating with multi-disciplinary teams to ensure seamless transitions and holistic planning.
  • Maintaining professional standards in case documentation and safeguarding practices.
  • Supporting team development through reflection, mentoring, and professional development.

Skills

  • Qualified Social Worker with current registration
  • Knowledge of Care Act 2014, Mental Capacity Act 2005
  • Experience with adults with learning disabilities, autism
  • Strong analytical and evidence-based assessment skills
  • Effective communication skills
  • Ability to manage varied caseload autonomously
  • Negotiation skills with service providers
  • Experience in supporting transitions and progression
  • Understanding of strengths-based practice frameworks
