What the Job Involves
This role involves assessing the needs of vulnerable adults and their carers, supporting them to arrange services to meet identified needs and outcomes. Key duties include promoting independence and recovery, ensuring timely care management assessments, fostering community-based service connections, addressing safeguarding procedures, managing risks, and maintaining high standards in service users' reviews and casework documentation. You will work closely with community teams and other professionals to provide holistic care.
