Who You Are

We are seeking an experienced worker with excellent organizational skills to assist our service in identifying and progressing case closures. You will work alongside the allocated worker and manager to ensure safe closures and step-down to universal services. Your experience and collaborative skills will be crucial in carrying a small caseload and helping to progress cases efficiently.

What the Job Involves

This role involves a variety of responsibilities aimed at ensuring the efficient closure of cases. You will be expected to:

• Work closely with the Group Manager, Team Managers, and practitioners to identify and progress case closures.
• Review all open cases in each team, particularly those marked as "closure ready" or experiencing delays.
• Meet with team managers and caseworkers to uncover barriers and identify quick wins for case progress.
• Compile prioritized lists of cases that could be closed with targeted intervention.
• Collaborate directly with families and professionals to address outstanding issues.
• Address administrative bottlenecks through effective communication with business support or management.
• Conduct weekly meetings to update on case progress, identify new cases, and share successful strategies.
• Track and report weekly case closures, documenting progress and highlighting any persistent obstacles.
• Collect feedback from caseworkers and families, recommending improvements for future case closures.
