Role

Who You Are

A proactive and experienced Senior Social Worker or Social Worker with a strong background in Adults Social Care. You are committed to supporting vulnerable adults and their carers, promoting independence, and adhering to safeguarding procedures. You possess excellent assessment skills and can work collaboratively in multi-disciplinary teams.

What the Job Involves

As part of the Short-Term Reviews Team, you will assess the needs of adults and their carers during a 6-month period, facilitating emergency and short-term care increases. You will ensure timely assessments, promote independence, and enable service users to access community services. Responsibilities include creating support plans, managing risks, safeguarding, and maintaining accurate records. You will work in partnership with community development staff, advocates, and carers, and participate in peer challenges to develop practice. Adhering to relevant legislation and policies is critical, as is maintaining Social Work England registration.
