Please Add JDSenior Family Practitioner – Pathfinder Families First Programme (Lewisham) Key Duties and Responsibilities Hold a caseload of Children in Need (CIN) cases, ensuring effective and timely interventions. Undertake regular home visits to assess the safety, needs, and well-being of children and their families. Conduct direct work with children, using a range of evidence-based interventions to promote positive outcomes. Chair CIN meetings, ensuring robust planning and the active engagement of families and partner agencies. Provide comprehensive family support, including guidance to strengthen parenting skills and resilience. Maintain up-to-date and accurate case records, ensuring all interventions and contacts are clearly documented. Prepare and present high-quality reports for meetings, reviews, and court proceedings as required. Develop and implement multi-agency plans, working collaboratively with professionals from health, education, and social care. Advocate for children and families, ensuring their voices are heard and their views are represented in decision-making processes. Identify and assess risks promptly, implementing safeguarding procedures in line with statutory guidance and best practice. Deliver group work or parenting programmes where appropriate, supporting families to achieve change. Respond effectively to crisis situations, providing practical and emotional support to families in need. Participate in reflective supervision, ongoing training, and team meetings to support professional development and continuous improvement. Promote equality, diversity, and inclusion at all times in work with children and families. Undertake any other related duties as required by the service to support the overall aims of the Pathfinder Families First Programme.
